76M came to OPD with C/O sustained injuries on left side of chest dur to RTA

A 76 year old male came to OPD with C/O sustained injuries on left side of the chest due to a RTA. 
CT brain:diffuse age related cerebral atrophy with small vessel ischemic changes and focal right extracranial soft tissue hematoma in Right frontal region. 
No H/O loss of consciousness
No H/O Headache
No H/O nausea and vomiting
No H/O seizures, diplopia and blurring of vision
No H/O urinary and fecal inconvenience
K/c/o Hypertension
N/K/c/o DM, Asthma, TB, epilepsy
PERSONAL HISTORY
Diet:mixed
Bowel movements:Regular
Addiction:
VITALS
BP:190/100
PR:80bpm
RB:BAE-positive, No added sounds, breath sounds slightly decreased on Rt lower lobes. 
HOPI
Raised BP is seen but gradually relived on medication(T.telma40) 

PROVISIONAL DIAGNOSIS:
Hypertension

The patient wanted to change the dosage of the medicine.
